Based on a contract year (12 months)
Premium is paid in full at the start of the 12 month contract.
Dental benefits include the following:
- Twice per contract year
- Periodic exam/comprehensive exam
- Hygiene visit including fluoride (does not include periodontal treatments)
- Once per contract year
- One set of bitewing x-rays (4 bitewings for adults, 2 bitewings for children)
- One emergency visit including one x-ray
- One panoramic x-ray
- 30% discount* off all dental services and procedures up to $2,500 discount per year
